Upper Cumberland Life & Health (“we,” “us”) respects your privacy. This page explains what information we collect through www.uclifeandhealth.com, how we use it, and the choices you have.

Information we collect

• Information you give us. When you fill out our contact form, call us, or email us, we collect what you share, such as your name, phone number, email address, ZIP code and your message.
• Newsletter sign-up. If you subscribe to our email newsletter, we collect your name and email address through our email service provider, Constant Contact. You can unsubscribe at any time using the link in every email.
• Website usage information. We use Google Analytics to see how visitors use the site, such as which pages are viewed, the type of device and browser, the general area and how you arrived. Google Analytics uses cookies, which do not identify you personally.
• Spam protection. Our contact form uses Google reCAPTCHA to block automated spam. reCAPTCHA collects hardware and software information and is governed by Google’s Privacy Policy and Terms of Service.

How we use your information

• To answer your questions and set up your free consultation.
• To send the newsletter you asked for.
• Through cookies, to understand which pages help visitors so we can improve the site.
• To protect the site from spam and misuse.

Who we share it with

We do not sell your personal information. We do not share your name, address, phone number or other personal details with anyone except an insurance carrier, and only when you ask us to quote, enroll or service a policy on your behalf.

Two things to know:

• If you subscribe to our newsletter, your name and email address go to Constant Contact, the service that sends it.
• Google provides cookie-based site analytics, which does not identify you personally, and spam protection on our contact form.

We may also disclose information when the law requires it.

Please do not send sensitive details through the website

Do not put Social Security numbers, bank account numbers, Medicare numbers or health details in the contact form or in an email. If we need those to help you, we will collect them by phone or in person.

Cookies

The site uses cookies for analytics and spam protection, as described above. You can block or delete cookies in your browser settings. The site will still work, although some features may not.

How long we keep it

We keep your information as long as we need it to serve you or to meet legal and insurance record-keeping requirements. You can ask us to delete any contact information you submitted, and we will do so.

• Unsubscribe from the newsletter using the link in any email.

If we change this policy we will post the new version here and update the date at the top.
